In an effort to improve access and accommodations for all CSAVR Conference Participants, we are requesting more specificity from conference participants, related to any accommodations you require, or would like to have, if they are available. Given that we are meeting in a hotel, the availability of accessible rooms and other physical accommodations are limited. With this in mind, please complete the following questionnaire with as much specificity with which you are comfortable. The information you provide will be kept private except when necessary to assure the requested accommodations are provided. For example, if you need wheel chair access, roll in showers, or Braille markings, we would ask that you list that under required. If there are other things that would make your stay more comfortable, but are not necessary, we ask that you list those under preferred. We will work closely with the hotel to try and meet all of the accommodation needs of our participants and thank you in advance for helping us work through this process. CSAVR cannot be responsible to provide an accommodation unless we know it is needed and we have time to make the necessary arrangements, so please complete the information requested.
